Complete a PPWR Verifeyer form in the ProductIP platform for each packaging unit you place on the EU market. The form is used to collect packaging information needed to support conformity assessment and technical documentation under Regulation (EU) 2025/40 on packaging and packaging waste (PPWR).

Before you begin
Ensure you have all relevant packaging data available before starting the form:
- BoM
- material datasheets
- test reports
- supplier information
- document IDs for evidence already uploaded in the Technical File
Evidence is not uploaded in the Smart Form. Please upload the relevant evidence in the Technical File and enter the related document ID in the Smart Form where requested. Acceptable evidence formats for upload in the Technical File are PDF, JPG, PNG and Excel for mass data. The maximum file size per upload is 10 MB.

Steps
- Go to the PPWR Verifeyer form. See Open a Verifeyer form to find out where to find the form.
- Start with the Introduction section and read the guidance.
- (Optional) Click Hide to hide guidance chapters "PPWR roles & responsibility” and “Packaging types & data guidance” to turn the form into a clean checklist.
- Read the DISCLAIMER section to acknowledge scope and responsibility.
- (Optional )Read "PPWR roles & responsibility" and "Packaging types & data guidance" to understand required input fields.
- Fill in "Actor & placing on the market", including your role in the supply chain. If you are based outside the EU, select the checkbox to appoint an "EU Authorised Representative" and provide their contact details.
- Complete Scope & context by filling in the required fields (marked with *). Confirm whether it contains multiple components.
- Click the Assessment Object button for each physical component (e.g., outer carton, plastic bag, EPS moulding). For each object, select the correct packaging category from the PPWR Annex II list.
- Work through the chapters below one by one. For every “Yes” answer supported by evidence, enter the related document ID in the Smart Form where requested (e.g. Test Report- SoC- V001.pdf).. The evidence itself must be uploaded in the Technical File.
- Review "EU DoC - Template" and "EPR Master Data Extract" for reference only (not part of final submission).
- Go to the Finalize section and fill in the required information to receive a PDF copy of the completed form.
- Sign the form and click the blue Finalize button to complete the questionnaire.
Tips and best practices
- First-time completion takes longer; plan 25–35 minutes per packaging type.
- Start with available data and leave gaps visible.
- Select “Unknown” instead of guessing.
- Prioritise missing information based on compliance risk.
- Work through each chapter step-by-step instead of skipping ahead.
- The two guidance chapters – “PPWR roles & responsibility” and “Packaging types & data guidance” can be hidden.
- Each question also includes short help (Why we ask / What it means / How to answer / Example), which remains visible while completing the form, but is not included in the finalised document.
- Use the Packaging types & data guidance to understand which category fits your material.
- Use consistent naming for documents and component IDs across chapters.
- In the top right corner you have the option to view and restore a previous draft, save or delete your draft and to see a preview of the final document.

Troubleshooting
- The questionnaire does not show questions for a specific article.
If no questions are displayed for a specific article, this means that the system does not identify this article as relevant for the selected packaging type.
If you believe the article is relevant, go back to Scope & Context and check whether the correct packaging type, use case and context have been selected. Once the relevant context is selected, the related questions should become available.
- I get a warning “Recyclability is currently unknown”.
A “No” answer indicates a potential recyclability compliance risk. Under Article 6(1) PPWR, all packaging placed on the market shall be recyclable. Further clarification with the responsible manufacturer is required, and the assessment must be reviewed again once the harmonised PPWR methodology applies.

FAQ
- Is it possible to save a draft and continue later?
Yes, you can click the Save icon in the top right corner to save your progress.
- Can I use this form for EPR reporting?
No. The PPWR Verifeyer form is not a complete EPR reporting tool. It collects packaging master data, such as material and weight information, which may also be useful for EPR processes. However, national EPR registrations, quantity reports, fees and country-specific reporting duties must be handled separately.
- Do I need to complete all sections?
Yes, unless a section is not applicable. Non-applicable sections should still be reviewed.
- What should I do if information is not known yet?
Do not guess. Select Unknown if this answer option is available. Add a short explanation in the Remarks field where needed. This helps to show missing data clearly.
- Can I upload evidence in the Smart Form?
No. Evidence documents are managed in the ProductIP Technical File. In the Smart Form, enter the requested data and, where relevant, the document reference or document ID.
- Can I edit the form after finalisation?
No. After finalisation, the form can no longer be edited. It is then a closed and submitted data record.
- Who is responsible for the submitted information?
The person finalising the form confirms the submitted information with a signature and takes responsibility for its correctness and completeness. ProductIP is not responsible for the content accuracy of the submitted information.
